Job summary

Your Health Organization in South Carolina is hiring a Registered Nurse for in-home Continuous Care in the Florence and Sumter area. The role involves intensive bedside care to manage symptoms and ensure comfort and safety, collaborating with an interdisciplinary hospice team.

The position requires daily travel, a 7-on/7-off, 12-hour shift schedule, and serves a full patient load across the region. We offer a competitive package with benefits and vehicle allowance.

Qualifications

• Must be a licensed nurse in good standing with the appropriate board.
• Registered nurse preferred with current license.
• Minimum three years clinical experience preferred.
• Experience in community/home-based settings preferred.
• Strong written and verbal communication abilities.
• Proficient with basic computer skills.
• Ability to lead and coordinate care teams.
• Demonstrates teamwork and sound judgment in varied conditions.
• Willingness to travel between offices/facilities and drive as required.

Responsibilities

• Facilitate receiving medical records from the patient’s primary provider and specialists.
• Review medical records and complete consents with patients.
• Enroll patients in Care Management and initiate a Plan of Care when eligible.
• Coordinate Care Management activities and document care plans.
• Evaluate for home health, hospice, palliative needs, or referrals as appropriate.
