Job summary

Your Health in Columbia, SC is seeking a dedicated Continuous CNA to support hospice patients across the Columbia area. The role involves home visits, collaboration with an interdisciplinary team, and travel to multiple service areas including Irmo, Columbia, and Blythewood.

You will perform personal care, monitor patient conditions, and facilitate telehealth visits while maintaining compliance with regulations.

Qualifications

• Certified Nursing Aide required. Certifications or licenses must be in good standing.
• Phlebotomy experience preferred. If not experienced, willing to train and demonstrate competency.
• Public health or home health experience preferred.
• Experience with DME delivery or logistics preferred.
• Proficient with technology tools for remote monitoring, telemedicine, and EMR.
• Strong organizational and time management skills; multitask effectively.
• Excellent reading and written/verbal communication skills.
• Basic computer knowledge.
• Ability to work independently with minimal supervision.
• Good interpersonal communication under all conditions.
• Flexibility to work hours and travel between locations.
• Licensed driver with insured vehicle in good condition.
• Clean driving record and professional vehicle operation experience.

Responsibilities

• Work with care team to ensure coordinated patient care per plan.
• Communicate via phone, email, Teams, Athena, and in person.
• Provide compassionate personal care for hospice patients at home.
• Assist with safe mobility, transfers, and fall prevention.
• Perform light housekeeping related to patient care (linens, areas, laundry).
• Monitor and document condition changes, skin integrity, appetite, comfort, safety.
• Report significant changes to hospice nurse or clinical team promptly.
• Support ADLs while promoting dignity, comfort, independence.
• Provide emotional support to patients and families.
• Assist with meals, hydration, and feeding per care plans.
• Deliver medications, review instructions, reconcile lists.
• Coordinate telehealth visits with pharmacists, specialists, and PCPs.
• Perform routine venipuncture and proper specimen handling.
