Job summary

Your Health Organization in Florence, SC is seeking a dedicated Continuous CNA to serve hospice patients in the Florence and Sumter area. The role involves traveling to patients' homes, providing compassionate personal care, monitoring conditions, and coordinating with the hospice team to maintain comfort and safety.

As a full-time CNA with phlebotomy and EMR skills, you will support ADLs, assist with medications and telehealth coordination, and document patient changes while upholding all laws

Qualifications

  • Certified Nursing Assistant with active license in good standing.
  • Phlebotomy experience preferred; willing to train if needed.
  • Experience in home health or public health preferred.
  • Proficiency with EMR, telemedicine, and remote monitoring tools.

Responsibilities

  • Provide compassionate personal care in patients' homes (bathing, grooming, hygiene).
  • Assist with mobility, transfers, and fall prevention.
  • Document patient conditions and report changes promptly.
  • Support ADLs and provide emotional support to patients and families.
  • Deliver medications, review instructions, and reconcile lists.
  • Coordinate telehealth visits with pharmacists and specialists.
  • Perform routine venipuncture and collect blood specimens as needed.

We are looking for a dedicated candidate join our team as a Continuous CNA) to service our hospice patients in the Florence and Sumter area. The Continuous (CNA) collaborates closely with the interdisciplinary hospice team to stabilize symptoms and ensure comfort, dignity, and safety. Provide quality healthcare in adherence to all applicable laws, regulations, and policies within the scope of practice.

The Continuous CNA will have a full patient load against all patients in the region based upon priority

Continuous CNA will perform care in homes in their designated service area. You must have reliable transportation as travel is required daily. This is a full time, salary-based working position

7 on 7 off 12/hr shifts

What Are We Looking For?

Areas of Responsibility:
  • Work closely with care team members and other staff to ensure coordinated patient care and compliance with the care plan.
  • Communicate effectively via phone, email, Microsoft Teams, Athena, in-person, etc.
  • Provide compassionate personal care to hospice patients in their homes, including bathing, grooming, oral hygiene, dressing, and toileting.
  • Assist patients with safe mobility, transferring, repositioning, and fall-prevention support.
  • Perform light housekeeping tasks related to patient care (changing linens, cleaning care areas, laundering patient items).
  • Monitor and document patient condition changes, including skin integrity, appetite, comfort levels, and safety concerns.
  • Report any significant changes in patient condition to the hospice nurse or clinical team promptly.
  • Support patients with activities of daily living (ADLs) while promoting dignity, comfort, and independence.
  • Provide emotional support and companionship to patients and their families.
  • Assist with meal preparation, feeding, and ensuring adequate hydration based on care plans.
  • Deliver medications to patients' homes, review instructions, and perform medication reconciliation, ensuring accuracy between current medication lists and what is found in the home.
  • Coordinate and facilitate telehealth visits between patients and pharmacists, specialists, therapists, and primary care providers, ensuring patients receive timely care.
  • Perform routine venipuncture for laboratory testing and ensure proper specimen handling and labeling.
  • Collect blood specimens during home or facility visits as needed to support patient care and diagnostics.
Qualifications:
  • Certified Nursing Aide required. Certifications or licenses must be in good standing.
  • Phlebotomy experience preferred. If not experienced in phlebotomy, candidate must be willing to complete training and demonstrate competency in the skill.
  • Previous experience in public health or home health preferred.
  • Experience with DME delivery or logistics operations preferred.
  • Must be proficient in using technology tools and platforms for remote patient monitoring, telemedicine, and electronic medical records.
  • Strong organizational and time management skills, with the ability to multitask and prioritize responsibilities effectively.
  • Ability to read and communicate effectively. Strong written and verbal communication skills.
  • Basic computer knowledge.
  • Must be able to work independently with minimal supervision and direction.
  • Should demonstrate good interpersonal and communication skills under all conditions and circumstances.
  • Must be able to work flexible hours and travel between offices, facilities, etc.
  • Must be a licensed driver with an automobile that is insured in accordance with state and/or organization requirements and is in good working condition.
  • Must have a clean driving record and experience operating vehicles in a professional capacity
