Your Health is seeking a dedicated Continuous CNA to serve hospice patients across the Grandstrand area, with a 7-on/7-off, 12-hour shift schedule. You will work closely with the hospice team to stabilize symptoms, ensure comfort, and maintain safety in patients' homes, requiring reliable transportation and daily travel.

The role emphasizes compassionate personal care, coordination with care teams, and adherence to regulations within scope of practice.

Qualificações

• Certified Nursing Assistant required and in good standing.
• Phlebotomy experience preferred; willing to train if needed.
• Public health or home health experience preferred.
• DME delivery or logistics experience preferred.
• Proficient with technology for remote patient monitoring, telemedicine, and EMR.
• Strong organizational and time management skills.
• Excellent written and verbal communication.
• Must be able to travel between offices and facilities.

Responsabilidades

• Collaborate with hospice care team to ensure coordinated patient care.
• Provide personal care (bathing, grooming, oral hygiene, dressing, toileting).
• Assist with mobility, transfers, repositioning, and fall prevention.
• Monitor and document patient condition changes and skin integrity.
• Deliver medications to patients’ homes and reconcile instructions.
• Coordinate telehealth visits with pharmacists, therapists, and providers.
• Perform routine venipuncture for lab testing.
• Travel daily within designated service areas.
