Job summary

Altus Hospice is seeking a self-driven Community Liaison to join our team, serving the Kingwood area. You will build trust-based relationships with referral sources and physicians to grow census and promote our end-of-life care services.

This role emphasizes compassionate outreach, education, and collaboration with healthcare partners. The position offers a Monday–Friday schedule (8 am–4:30 pm) with potential for growth within a mission-driven, patient-centered organization.

Qualifications

• Bachelor’s degree or equivalent in a related field; healthcare experience preferred.
• Minimum two years in healthcare sales/marketing; hospice/palliative marketing preferred.
• Proven ability to establish and maintain relationships with physicians, healthcare professionals, and accounts.

Responsibilities

• Establish and nurture relationships with physicians and healthcare partners to build collaboration and trust.
• Advocate for hospice services through outreach reflecting compassionate, high-quality care.
• Grow presence by positioning Altus Hospice as the provider of choice across the service area and drive census growth.
• Drive referral growth through regular visits with referring physicians, discharge planners, SNFs, social workers, and other professionals.
• Lead initial conversations with patients and families about hospice care and obtain informed consent empathetically.
