Job summary

Altus Hospice is seeking a self-driven, compassionate Community Liaison to join our Kingwood area team. You’ll be the face of Altus Hospice, building trust-based relationships with referral sources, physicians, and healthcare partners to promote our hospice services and grow census.

This role offers a Monday–Friday schedule, territory in Tomball and Willowbrook, and a comprehensive benefits package including health insurance, PTO, tuition assistance, and a 401(k) match.

Qualifications

• Bachelor’s degree preferred; additional experience may substitute.
• Minimum two years of sales/marketing experience in healthcare.
• Experience establishing relationships with physicians, healthcare professionals, and community agencies.

Responsibilities

• Establish and nurture relationships with key decision-makers, including physicians and healthcare partners.
• Serve as a passionate advocate for hospice services through regular outreach and high-quality care.
• Grow presence by promoting Altus Hospice as the provider of choice across the service area.
• Drive referral growth through regular visits with referring physicians, discharge planners, SNFs, social workers, and other professionals.
• Support patients and families by leading initial conversations about hospice care with empathy.
