Job summary

Altus Hospice Care seeks a self-driven Community Liaison to champion hospice services in the Alice area. You will build trust-based relationships with physicians, referral sources, and healthcare partners to enhance access to compassionate care.

Responsibilities include outreach, education, and driving census growth while representing Altus Hospice Care with empathy and professionalism in the community.

Qualifications

• Bachelor’s degree in related field preferred; experience may substitute.
• Minimum two years of sales/marketing experience in healthcare; hospice experience preferred.
• Skilled at establishing relationships with staff, marketing contracts, and clinical staff to ensure understanding of Hospice services.
• Proven ability to develop relationships with new and existing accounts including physicians and healthcare professionals.
• Prior clinical experience is a plus.
• Must have reliable transportation, valid license, and auto insurance.

Responsibilities

• Establish and nurture relationships with key decision-makers, including physicians and healthcare partners, to strengthen collaboration and trust within the medical community.
• Conduct regular outreach reflecting commitment to compassionate, high-quality care and drive census growth.
• Grow presence by positioning Altus Hospice as the provider of choice across the service area and exceeding growth goals.
• Drive referral growth through regular visits with referring physicians, discharge planners, SNFs, social workers, and other healthcare professionals.
• Lead initial conversations about hospice care, explain services with empathy, and facilitate consent.
