Job summary

Agape Hospice & Palliative Care in Torrance, CA is seeking a Hospice Community Liaison to grow relationships with physicians, hospitals, skilled nursing facilities, home health agencies, and community organizations. You will educate them about hospice and palliative care eligibility with honesty and compassion.

You will work closely with our clinical and intake teams to ensure referrals are appropriate and timely, represent Agape at events and professional meetings, and maintain accurate

Qualifications

• 2+ years of hospice or palliative care community liaison, outreach, or business development experience
• Established relationships with referral sources in Los Angeles and/or Orange County
• Strong working knowledge of hospice eligibility and the Medicare Hospice Benefit
• Warmth, patience, and the ability to communicate with families with sensitivity
• Unquestioned integrity and a commitment to compliance
• Valid California driver’s license, reliable transportation, and current auto insurance

Responsibilities

• Provide accurate education on hospice and palliative care eligibility, levels of care, and the Medicare Hospice Benefit
• Meet with patients and families to explain services, answer questions, and support informed decision–making, without pressure
• Coordinate with intake and clinical staff so referrals are evaluated promptly and appropriately
• Represent Agape at community events, in-services, and professional meetings
• Maintain accurate records of outreach activity and adhere to all federal and California compliance requirements, including anti-kickback and patient-choice rules

Agape Hospice & Palliative Care is an owner-operated, Torrance-based hospice serving patients and families throughout Los Angeles and Orange County since 2014. We are known for careful clinical practice, strict regulatory compliance, and a team that treats every patient and family with dignity. We're growing thoughtfully, and we're looking for a Hospice Community Liaison who wants to grow with us.

The role

The Hospice Community Liaison is the face of Agape in the community. You'll build and maintain relationships with physicians, hospitals, skilled nursing and assisted living communities, home health agencies, and community organizations; educate them about hospice and palliative care eligibility and services; and help patients and families understand their options with honesty and compassion. You'll work closely with our clinical and intake teams to ensure every referral is appropriate, timely, and handled with care.

Compensation and benefits

Base salary of $85,000 to $130,000 per year. Placement within the range is based on hospice and palliative care experience, territory knowledge, and the strength of existing professional relationships. A performance bonus program rewards outreach activity, education, and quality measures. A full benefits package is offered; details are provided during the interview process.
