Agape Care Group is seeking a Clinical Liaison to coordinate and grow hospice referrals, build relationships with hospitals, and educate community and referral sources about hospice services. The role emphasizes continuity of care and clear communication with patients and care teams.

The candidate will work to identify new markets, coordinate care for referred patients, and collaborate with agency staff to ensure smooth operations and documentation.

Qualifications

• Requires nursing degree and licensure as applicable.
• Minimum 2 years of clinical care experience; hospice experience preferred.
• Must have reliable transportation and ability to lift up to 25 lbs.

Responsibilities

• Coordinate and generate hospice business in existing and new accounts; identify new markets and maintain services.

Essential Functions

The Clinical Liaison is responsible for coordinating & generating new hospice business in both existing and new accounts, identifying new markets and maintaining existing service as well as coordinating care for referred patients. The Clinical Liaison's primary responsibility is serving as a liaison between the agency, hospitals, medical community and other referral sources. This position educates the community and the medical profession/referral sources regarding hospice services. This position involves daily interaction with patients, medical professionals, other referral sources, and the community to assure continuity of care and to coordinate appropriate communication and documentation. This position works closely with agency personnel to ensure that patient care is handled efficiently and effectively. The Clinical Liaison is regularly engaged away from the employee’s place of business in performing these duties.

Qualifications

• A heart to serve patients and families and a passion for providing the best possible care
• Education: Graduate of an accredited school of nursing
• Experience: 2 year of experience in a clinical care setting, hospice preferred
• Required: Reliable transportation. Ability to sit, stand, bend, move intermittently and lift at least 25lbs and bear the weight of an average adult effectively.
