Qualifications

• Strong communication and interpersonal skills for compassionate patient support.
• Ability to work independently and within an interdisciplinary team.
• Proficiency with basic computer/tech tools used for documentation.

Responsibilities

• Provide personal care, hygiene support, and mobility assistance per plan.
• Record vital signs and report changes to the RN Case Manager.
• Assist with meals, laundry, and household tasks to support care.
• Participate in weekday visits with occasional weekend rotation as needed.

Education

State‑approved CNA program with active CNA certification
