PACS is seeking a dedicated Certified Nurse Aide for hospice care. The role supports patients and families with dignity, empathy, and professionalism, ensuring comfort and quality of life during a meaningful stage of care. You will provide personal care and record vital signs in a supportive home setting.

Competenze

• Strong communication and interpersonal skills to support patients and families.
• Ability to work with an interdisciplinary team and independently as needed.
• Proficiency with basic computer tools used for documentation.

Mansioni

• Provide personal care and hygiene support and patient ambulation.
• Take and record vital signs and clinical metrics as directed.
• Observe, document, and report changes to the RN Case Manager.
• Participate in scheduled weekday visits and maintain a consistent schedule.
