Advantage Home Health and Hospice in Berks County, PA is seeking a dedicated Certified Nursing Assistant (CNA) to join our hospice care team. You will provide compassionate in-home patient care, support families, and help manage daily activities.

Qualifications

• Valid Certified Nursing Assistant (CNA) certification.
• Valid Driver's license and state required Auto Insurance.

• Understanding of end-of-life care principles; hospice experience a plus.

Responsibilities

• Deliver empathetic and compassionate care to patients in their homes or facilities.
• Assist patients with activities of daily living, pain management, and symptom control.
• Provide emotional support and companionship to patients and their families.
• Coordinate care with the interdisciplinary hospice team.
• Maintain accurate patient records and documentation.
• Ensure a peaceful and comfortable environment for patients.
• Advocate for patients and families.
