Responsibilities

• Provide personal care services including bathing, grooming, oral hygiene, and dressing for hospice patients.
• Assist with mobility, transfers, and positioning for comfort and safety.
• Monitor and report changes in patient condition and vital signs to the supervising nurse.
• Deliver compassionate emotional support to patients and families during end-of-life care.
• Assist with light housekeeping, meals, and other activities of daily living (ADLs).
• Document patient care activities accurately per hospice standards.
• Collaborate with the hospice care team for holistic, patient-centered care.

Responsibilities

• Provide personal care services including bathing, grooming, oral hygiene, and dressing for hospice patients
• Assist with mobility, transfers, and positioning to ensure patient comfort and safety
• Monitor and report changes in patient condition, vital signs, and overall well-being to the supervising nurse
• Deliver compassionate emotional support to patients and their families throughout the end-of-life journey
• Assist with light housekeeping, meal preparation, and other activities of daily living (ADLs)
• Document patient care activities accurately and in compliance with hospice care standards
• Collaborate with the interdisciplinary hospice care team to ensure a holistic, patient-centered approach

Requirements

• Active Certified Nursing Assistant (CNA) license in the state of Arizona
• Prior experience in hospice, palliative care, or home health preferred
• Strong understanding of end-of-life care, comfort measures, and patient dignity
• Excellent communication and interpersonal skills with patients and families
• Reliable transportation and ability to travel to patient homes in the Scottsdale, AZ area
• Ability to work independently with compassion, patience, and professionalism
• Current CPR certification and ability to pass a background check
