Job summary

Care Plus Hospice Care LLC in Phoenix, AZ seeks a compassionate Hospice CNA to provide direct patient care in home settings. Responsibilities include bathing, grooming, monitoring condition, and collaborating with the care team to ensure dignity and comfort.

Must hold Arizona CNA license, CPR, and reliable transportation; hospice/palliative experience preferred. Flexible schedule and training opportunities offered.

Qualifications

• Active Arizona CNA license.
• Experience in hospice, palliative care, or home health preferred.
• CPR certification and ability to pass a background check.
• Excellent communication and interpersonal skills with patients and families.
• Reliable transportation to travel to patient homes in Scottsdale, AZ.

Responsibilities

• Provide personal care services including bathing, grooming, oral hygiene, and dressing for hospice patients.
• Assist with mobility, transfers, and positioning for comfort and safety.
• Monitor and report changes in patient condition and vital signs to the supervising nurse.
• Deliver compassionate emotional support to patients and families during end-of-life care.
• Assist with light housekeeping, meals, and other activities of daily living (ADLs).
• Document patient care activities accurately per hospice standards.
• Collaborate with the hospice care team for holistic, patient-centered care.
