Job summary

Care Plus Hospice Care LLC in Scottsdale, AZ is seeking a compassionate Hospice Liaison / Care Coordinator to bridge patients, families, and providers for seamless hospice transitions.

You will build referral networks, educate on eligibility, coordinate timely evaluations, and support families through the hospice journey with a patient-centered approach.

Qualifications

• Prior experience in hospice, palliative care, home health, or related healthcare setting preferred.
• Proven background in healthcare sales, community outreach, or clinical liaison roles.
• Strong knowledge of hospice regulations, Medicare/Medicaid eligibility criteria, and end-of-life care principles.
• Excellent interpersonal and communication skills with a compassionate, patient-centered approach.
• Self-motivated, organized, and able to manage a territory independently.
• Valid driver's license and reliable transportation for local travel in Scottsdale area.

Responsibilities

• Build and maintain relationships with hospitals, SNFs, physicians, and other referral sources to drive hospice admissions.
• Educate healthcare professionals, patients, and families on hospice eligibility, services, and benefits of end-of-life care.
• Assist with coordination of timely hospice evaluations and admissions.
• Collaborate with interdisciplinary care team to ensure smooth patient transitions and continuity of care.
• Serve as a community ambassador at outreach events and networking opportunities.
• Maintain accurate documentation of referral activity, patient interactions, and care coordination efforts.
• Provide ongoing support and communication to families navigating the hospice process.
