Job summary

AccentCare is seeking a compassionate caregiver to support patients in accordance with the interdisciplinary plan of care. You will provide personal care, assist with turning, seating, standing, and basic mobility, and help with meal preparation and light housekeeping as needed.

Responsibilities include documenting care, reporting changes to the RN or TD, and participating in ongoing education and patient conferences to ensure high-quality, compassionate care.

Responsibilities

• Follow the interdisciplinary plan of care, be alert to the patient's needs and concerns.
• Provide personal care, including bathing, dressing, grooming, hair, nail, skin and oral hygiene.
• Assist patients, according to the plan of care in: Turning, Sitting, Standing.
• Ambulation with elimination, including: facilitation of bedpans, and urinals; assisting patient to the bathroom; routine Foley care; Enemas; meals, when necessary.
• Perform simple procedures as an extension of therapeutic services, in accordance with the interdisciplinary plan of care.
• Observe and report changes in the patient's condition and needs to the Team Director (TD) or Registered Nurse (RN).
• May perform simple homemaking chores for the patient directly, meal preparation, and/or light housekeeping in the patient's immediate area.
• Complete required documentation per policy.
• Participate in continuing education programs.
• Follow confidentiality regarding patients and families per policy.
• Demonstrate a compassionate attitude toward patients and families consistent with the philosophy and mission of the organization.
• Attend patient care conferences as requested by the TD.
• Perform other related duties and special projects, as assigned.
• Perform other duties as assigned including those within the scope of current, valid professional licensure/credentials.
