Job summary

Lower Cape Fear LifeCare in North Carolina is seeking a compassionate Hospice CNA to provide bedside care in patients' homes and varied care facilities. You will perform bathing, peri-care, dressing, mouth care, toileting assistance, and meal support while maintaining patient comfort and dignity.

You will document care on a mobile device, coordinate visits with RN case managers, train family caregivers, and ensure a steady supply of in-vehicle materials.

Qualifications

• High school diploma or equivalent required with CNA course completion.
• Current CNA certification and ability to work in home health/hospice settings.
• Roll-out training and competency assessed by RN; in-home evaluations occur periodically.

Responsibilities

• You will provide direct patient care including bathing, peri-care, dressing, and assistance with meals.
• You will document care at the bedside using a company mobile device and chart within facilities as needed.
• You will spend time with patients in their homes or care facilities and maintain cleanliness of the area and linens.
• You will drive between patients with mileage reimbursement and keep supplies stocked in your vehicle.
• You will work closely with RN case managers to communicate patient status and concerns, and train caregivers as appropriate.
• You will coordinate visit times and manage scheduling with patients or caregivers.
