Job summary
Three Oaks Hospice, Inc. is seeking a compassionate Certified Nursing Assistant (CNA) to join our team and provide in-home care for patients and families in Colorado.
The CNA will assist with personal healthcare, help create a safe home environment, and collaborate with the supervising RN to report observations. Ideal candidates will have CNA certification, state-approved nurse aide training, a current driver’s license, and reliable transportation.
Qualifications
- High School Diploma preferred.
- Has successfully completed Nurse aide training/competency program approved by the state and listed in good standing on the state nurse aide registry; or State Licensure program that meets requirements.
- Must have CNA license.
- Must have current driver’s license.
- At least one year experience, preferred, as a Hospice Aide or Nursing Assistant in a hospital, nursing home, home health agency, hospice, or long-term care facility.
- Reliable transportation.

- Position summary: provide personal healthcare and related services to patients in their place of residence; maintain a safe, clean environment; work cooperatively with patient and family; share observations with supervisor; operate under the direction of a registered nurse.
