Job summary

Three Oaks Hospice is seeking a passionate Certified Nursing Assistant (CNA) to join our team of hospice care professionals and make a meaningful difference through listening, caring, and serving patients at home. The role focuses on providing essential personal healthcare and maintaining a safe, clean home environment under RN supervision.

Responsibilities include patient-focused care, observation reporting, and collaboration with families and the care team.

Qualifications

• High School Diploma preferred.
• Has successfully completed Nurse aide training/competency program approved by the state and listed in good standing on the state nurse aide registry; or State Licensure program that meets requirements.
• Must have CNA license.
• Must have current driver’s license.
• At least one year experience, preferred, as a Hospice Aide or Nursing Assistant in a hospital, nursing home, home health agency, hospice, or long-term care facility.
• Reliable transportation and auto liability insurance.

Responsibilities

• POSITION SUMMARY: Primary function is to provide personal healthcare and related services to the patient in their place of residence.
• Assist in providing a safe and clean environment; work cooperatively with patient and family and share observations with the supervisor.
• Operate under the direction and supervision of a registered nurse.
