Job summary

Interim HealthCare of Fort Collins is seeking a compassionate Certified Nursing Assistant for a temporary hospice home-visit role. You will provide comfort and dignity to patients, support families, and work with a healthcare team to ensure excellent care.

The position requires a Colorado CNA license, at least 6 months of CNA experience, CPR/BLS, and reliable transport for travel between visits. Flu vaccination proof may be required during flu season.

Qualifications

• Active Colorado CNA license.
• Minimum 6 months CNA experience.
• Hospice experience preferred.
• Current CPR/BLS certification.
• Reliable transportation to travel between visits.
• Proof of flu vaccination during Oct–Mar.

Responsibilities

• Care for personal care and hygiene needs of hospice patients during home visits.
• Collaborate with a team of clinicians to deliver high-quality patient care.
• Assist with bathing, dressing, grooming, feeding and nutrition.
• Support with ambulation, transfers and ROM exercises.
• Check vitals, document changes, and monitor glucose as needed.
• Notify supervisor of any changes requiring care adjustments.
