Qualifications

• Licensed CNA in Texas required.
• 1+ years' experience preferred.
• Experience in Hospice or Home Health preferred.
• Effective written and verbal communication.
• Ability to travel with reliable transportation.
• Clean background check and drug screen.

Responsibilities

• Provide hands-on ADL assistance (bathing, dressing, grooming).
• Document changes in patient condition and report to nurse.
• Assist in medication administration as prescribed and record results.
• Maintain a clean, safe, and comfortable home environment.
• Support pain management per care plan.
• Maintain detailed patient records and observations.
• Collaborate with interdisciplinary hospice team and participate in care planning.
