Home Health in Colorado is seeking a dedicated Hospice CNA to provide compassionate end-of-life care in patients' homes. You will assist with hygiene, monitor patients, administer treatments as directed, and document care accurately for insurance and quality purposes.

The role requires a valid CNA license, 2+ years hospice experience, and excellent communication with patients, families, and the healthcare team.

Qualifications

Valid CNA license in the state of practice.
2+ years of hospice or palliative care experience required.
Home Health or Hospice exposure preferred.
Knowledge of end-of-life care principles.
Strong clinical skills with compassionate care.
Excellent communication and interpersonal skills.
Ability to prioritize tasks in a fast-paced setting.
Ability to build rapport with patients and families.

Responsabilités

Assist patients with personal hygiene, including bathing, grooming, and dressing.
Monitor vital signs and report any changes to the supervising nurse.
Administer medications and treatments as directed by the healthcare team.
Provide companionship and emotional support to patients.
Assist with mobility and positioning to enhance comfort and safety.
Support families in understanding and coping with end-of-life care.
Maintain a clean and safe environment for patients.
Accurately document care provided and report significant changes in the patient’s condition.
Adhere to all safety protocols and infection control measures.
Collaborate with family members and health care teams to ensure continuity of care.
Adhere to infection control and safety procedures to protect patients and staff.
Perform all other duties assigned.
