Job summary

Autumn Hospice, part of the Ascend family, is hiring a Certified Nursing Assistant (CNA) for the South Team in Weymouth, MA. Join a compassionate hospice program dedicated to maintaining patient dignity and comfort at end of life.

You will assist with ADLs, transfers, light housekeeping, meals, and vital signs, while collaborating with the Hospice Nurse and care team. A valid CNA certification and willingness to travel are required; a driver's license is essential.

Qualifications

• Must have a current state CNA certification.
• One year CNA experience preferred; hospice/home health experience preferred.
• Willing to travel throughout the assigned territory and to accompany patients.
• Valid driver’s license and reliable transportation required.

Responsibilities

• Assist with ADLs: personal hygiene, grooming, mouth care, skin care, nails.
• Assist with toileting, transfers, and turning bed-bound patients.
• Support prescribed exercises and ROM as directed by the Hospice Nurse.
• Assist with light housekeeping and changing bed linens.
• Feed and assist patients with meals.
• Take temperatures, pulse, and respirations as directed by the Hospice Nurse.
• Report changes in patient condition to the Hospice Nurse and document appropriately.
• Assist with medication reminders in compliance with regulations.
• Provide compassionate, respectful end-of-life care and communicate with the care team.
