Job summary

Ascend Hospice & Palliative Care MA is hiring a Certified Home Health Aide (CHHA) to support patients in their homes and facilities. The role includes assisting with daily living activities, mobility, medication reminders, and light housekeeping, under Hospice Nurse supervision.

Willingness to travel and a valid driver’s license are required. We value compassionate care and reliability as essential qualities.

Qualifications

• Must have a State Certified CHHA license
• Hospice or home health experience preferred
• Willing to travel to patient residences
• Valid driver's license and reliable transportation

Responsibilities

• Provide or assist with ADL activities including personal hygiene, grooming, skin care and nails
• Assist with toileting, positioning, transfer, and bed mobility
• Assist with prescribed exercises and ROM as instructed
• Assist with light housekeeping and patient laundry
• Feed and assist with meals
• Record vital signs under supervision and report changes to Hospice Nurse
• Administer or remind patient to take medications in compliance with regulations

Job description

Now Hiring -Certified Home Health Aide (CHHA)

ThisCertified Home Health Aidewill be responsible for, but not limited to:

1. Provides or assists in ADL activities; personal hygiene, grooming, mouth, skin care and nails.
2. Provides assistance in toileting; positioning and transfer activities, frequent turning and positioning of a bed bound resident/patient.
3. Assists with the prescribed exercise and ROM activities which the resident/patient and Hospice aide have been taught by the Hospice Nurse.
4. Assists with light housekeeping of individual's personal unit or room, including changing of bed linen, if needed and patient laundry.
5. Feeds and assists with meals.
6. Obtains temperature, pulse and respirations as supervised by the Hospice Nurse.
7. Reports any changes in resident's/patient's condition to the Hospice Nurse and, if in a facility, to the facility nurse and documents the information on the Home Health Aide Certification of House form, including the date and time of the report and the person receiving the information.
8. Assists or reminds the patient to take medication, in compliance with regulations of appropriate regulatory and accrediting bodies.

Education:

High School Diploma or equivalent

Professional Experience:

• Must have a State Certified CHHA license
• Experience in hospice or home health, preferred
• No experience necessary but 1 year, preferred
• Must be willing to travel
• Must have a valid driver's license and reliable transportation
