Job summary

Cantex Continuing Care Network seeks a compassionate CNA to provide personal care and supportive services to hospice patients in their homes or facilities. You will follow the patient’s plan of care under nurse supervision, promoting comfort, dignity, and safety.

You will assist with ADLs, document observations, and report changes to the clinical team. Reliable transportation and English communication are essential.

Qualifications

• Current state certification as a CNA required.
• Hospice, home health, long-term care experience preferred.
• Strong communication and patient care skills.
• Ability to follow patient care plans and nursing staff instructions.
• Reliable transportation, valid driver’s license, and current auto liability insurance.
• Ability to read, write, and communicate effectively in English.

Responsibilities

• Provide personal care services including bathing, grooming, dressing, toileting, and mobility assistance.
• Assist patients with activities of daily living (ADLs) according to the plan of care.
• Document patient care and observations accurately and timely.
• Report changes in patient condition or environment to supervising nurse.
• Promote patient safety, comfort, dignity, and independence.
• Maintain a clean and safe patient environment.
• Follow infection control and safety procedures in all patient care activities.
• Assist with safe transfers and mobility using proper lifting techniques.
• Attend in-service training and maintain CNA certification requirements.
• Communicate effectively with patients, families, nurses, and interdisciplinary team members.
