Qualifications

• HS Degree or GED required.
• Proof of State Certification - Certified Nurse Aide required.
• 1 year of direct patient care experience required.
• Must demonstrate skills and written competency before working independently with the patient.

Responsibilities

• Provides personal hygiene care to patients according to the individual Plan of Care, such as bathing and shampooing.
• Deliver routine medical supplies as needed to patients.
• Observe and report any changes in the patient's condition to the Nurse/Care Manager.
• Demonstrate respect and understanding of CHOT's philosophy, mission, and service standards, and provide excellent customer service.
• Complete 12 hours of in-service training annually.
• May assist the patient with transfers as needed.
• Provide compassionate companionship to patients while delivering services.
• Assignments may include patients in the field or in inpatient units.
