Job summary

Constellation Hospice is seeking a Certified Nursing Assistant (CNA) to support our Hospice Program. The CNA assists with personal care, grooming, and daily living activities for patients, working closely with families and the supervising nurse.

Responsibilities include performing bed, sponge, and shower baths, hair washing, and following physician orders. A valid CNA license and driver’s license are required, with hospice/home health experience preferred.

Qualifications

• Must have completed the CNA training and competency evaluation program approved by the state.
• CNA License must be active and in good standing.
• Must have a current driver's license and be willing to drive within an assigned territory.
• Must have reliable transportation.
• At least one year of hospice/home health experience preferred.
• Basic knowledge of iOS operating system.

Responsibilities

• Assist with personal care and provide assistance to patients and families.
• Work cooperatively with patients and their families.
• Provide grooming and hygiene assistance (bed bath, sponge bath, shower bath, hair shampooing).
• Perform tasks under physician orders and supervision of a registered nurse.
