Job summary

Heart to Heart Hospice is seeking a compassionate Hospice Aide (CNA) to provide direct patient care under RN supervision in Indiana. You will assist with transfers, document care, and maintain infection control while communicating effectively with patients and families.

Requirements include a current CNA license, High School Diploma or GED, at least one year of relevant experience, and a valid driver's license. We offer competitive benefits and a company car for qualifying individuals.

Qualifications

• High School Diploma or GED preferred.
• Unencumbered CNA license per Nurse Aide Registry.
• Current CNA license.
• At least one year of Hospice Aide or Nursing Assistant experience.
• Demonstrates interest in welfare of ill and elderly; compassionate care.
• High integrity and confidentiality.
• EMR experience a plus.
• Must have valid driver's license and auto liability insurance.

Responsibilities

• Provide direct patient care under RN supervision and according to the Plan of Care.
• Assist patient with transfers and ambulation per agency policy.
• Report and document changes and care provided to ensure continuity of care.
• Practice infection control principles and uphold safety standards.
• Promote respectful communication with patient, family, and staff.
• Submit all documentation promptly and accurately.
• Perform other duties as required and ensure compliance with laws and policies.
