Acara Hospice is seeking a dedicated Hospice Aide (CNA) to provide direct, hands-on care to patients in their homes. This full-time role involves assisting with personal care, maintaining a safe environment, and supporting families as part of a compassionate, interdisciplinary team.

The ideal candidate holds a Texas CNA certification, is experienced in hospice or home health settings, and possesses strong communication and teamwork skills.

Qualifications

• CNA certification required and applicable for direct patient care.
• Strong communication and interpersonal skills for teamwork with families and staff.
• Ability to perform safe patient transfers and mobility assistance; follow infection control protocols.
• Texas CNA certification, valid driver’s license and reliable transportation; hospice/home health or long-term care experience preferred.

Responsibilities

• Assist patients with personal care including bathing, grooming, dressing, toileting and mobility.
• Monitor patient condition and report changes to the nursing team.
• Maintain a safe, comfortable home environment and respect patient dignity.
• Support families and collaborate with interdisciplinary team on care plans.
• Travel to patient homes as part of regular hospice visits.
