Job summary

Community Healthcare of Texas in Cleburne, TX is seeking a Certified Nurse Aide to provide personal hygiene care to hospice patients as part of the Plan of Care.

Responsibilities

• Provide personal hygiene care to hospice patients according to the Plan of Care.
• Deliver routine medical supplies to patients as needed.
• Observe and report changes in patient condition to the Nurse/Care Manager.
• Demonstrate respect for CHOT's philosophy and service standards; provide excellent customer service.
• Complete 12 hours of in-service training annually.
• May assist with patient transfers as needed.
• Provide compassionate companionship during service delivery.
• Assignments may be in the field or in inpatient units depending on census.
