VIA Health Partners is a community-based, not-for-profit hospice and palliative care provider in Charlotte, NC. We seek compassionate CNAs to deliver high-quality patient care, including ADLs, bathing, nutrition, and safety. The role emphasizes teamwork with IDT and adherence to HPCCR guidelines.

Formación

• High School diploma preferred, plus training/certification in related field required.
• Certified Nursing Assistant; maintain current listing status required.
• Must be in good standing with NC DHHS.

Responsabilidades

• Delivery of patient care and ADL support.
• Assist with bathing, feeding, mobility, and personal care.
• Maintain infection control and safety protocols.
• Participate in interdisciplinary team conferences (IDT).
• Ensure accurate clinical documentation according to guidelines.
• Maintain CPR certification and pursue ongoing professional development.
