Prismhr Hire seeks a compassionate Certified Nursing Assistant to join our Hospice Interdisciplinary Team in Brunswick, GA. Under the Director of Clinical Services, you’ll provide personal care, assist with daily living activities, and help maintain a safe, comfortable environment for terminally ill patients.

You will document duties in the EMR, support families, observe changes, and follow the hospice plan of care and regulatory requirements.

Qualifications

• Must possess a current Georgia CNA license/certificate.
• Must maintain a current CPR certification.
• Must possess a Georgia Driver’s license and proof of insurance.

Responsabilités

• Assist patients with activities of daily living and light housekeeping.
• Provide emotional support to patients and families.
• Observe changes and report to the primary nurse.
• Document daily duties in the EMR.
• Maintain safety in the patient environment.
• Follow the hospice plan of care and policies.
