Job summary

Responsibilities

• Provides personal care to hospice patients as prescribed and in accordance with care plans.
• Assists with activities of daily living: bathing, grooming, mouth care, and mobility.
• Measures and records vitals when ordered and documents patient status.
• Assists with meals, feeding if appropriate, and hydration.
• Assists with household tasks essential to patient health and safety.
• Documents services provided and reports changes to the RN or Nurse Manager.
