Job summary

Transitions Hospice in Peoria, IL is seeking a compassionate CNA to join our interdisciplinary care team. You will provide personal care, monitor vital signs, and support patients and families in their homes with dignity and respect.

Qualifications

What You'll Do

As a Transitions Hospice CNA, you're a vital member of the interdisciplinary care team — not just a support role, but a trusted presence for patients and families. Your responsibilities include:

• Providing personal care with dignity, comfort, and respect at the center of everything
• Attending to patient needs and requests promptly and compassionately
• Monitoring and recording vital signs (temperature, pulse, respiration, blood pressure) as directed
• Observing patients and reporting changes to the care team accurately and in a timely manner
• Communicating with patients and families in a positive, ethical, and confidential manner
• Maintaining thorough documentation in line with organizational standards
• Upholding the hospice philosophy with professionalism and genuine compassion
