Job summary

Residential Home Health and Hospice is seeking a dedicated Certified Nursing Assistant for hospice care in the Hershey, PA area. You will deliver compassionate, patient-centered care at patients’ homes, collaborating with an interdisciplinary team to ensure comfort and dignity.

You will perform ADLs, support families, document care in the EMR, and travel within your territory as needed. A CNA certificate, HS diploma, and a valid driver's license are required; hospice experience is preferred.

Qualifications

• Active CNA certificate in the state you work
• High School Diploma or certificate
• Minimum one year healthcare experience, Hospice experience preferred
• Current driver’s license and ability to travel ~20% of each day

Responsibilities

• Deliver compassionate, patient-centered care in a hospice setting with an interdisciplinary team
• Provide hands-on care to patients, assisting with ADLs
• Support patients and caregivers by reinforcing the care plan and offering emotional support
• Respond quickly to patient needs and document care in the EMR
• Travel to patient homes within assigned territory
