Qualifications
- Active CNA certificate valid in the state of employment.
- High School Diploma or equivalent.
- Minimum one year healthcare experience; hospice experience preferred.
- Valid driver's license and ability to drive ~20% of the day.
Responsibilities
- Deliver compassionate, patient-centered care in a hospice setting, assisting with ADLs.
- Observe and document changes in patient condition in the EMR.
- Provide care and reinforce the care plan to patients and caregivers.
- Travel to patient homes within assigned territory.
