Job summary

Amedisys is hiring a full-time hospice aide for the Oshkosh/Appleton territory in Wisconsin. You will provide compassionate in-home care, assist with daily living activities, and support patients in a home setting. The role emphasizes safety, nutrition, and collaboration with registered nurses and therapists.

Responsibilities include personal care, meal prep, light homemaking, and documenting changes in patient status. CNA certification, CPR, and a valid driver’s license are required.

Qualifications

• High school diploma or equivalent.
• Current state certification (or registration) as a nurse assistant per state requirements.
• CNA experience within the last two years.
• Current CPR certification.
• Valid driver’s license, reliable transportation and liability insurance.

Responsibilities

• Provides personal care services to patients.
• Applies safety principles and proper body mechanics to personal and supportive care tasks.
• Plans and prepares nutritious meals, assists with feeding, and performs homemaking tasks as assigned.
• Observes, reports and documents changes in patient’s status and reports to the primary nurse as needed.
• Follows written instructions for personal care prepared by a registered nurse.
• Assists patients with prescribed exercises as instructed by a nurse or therapist.
• Performs and records simple urine tests for sugar, acetone, and albumin.
• Other duties as assigned.
