Job summary

VNS Health's Hospice Care Program is seeking a capable manager to coordinate interdisciplinary teams delivering compassionate hospice services to patients and families. You will oversee operations, staffing, and performance, ensuring compliance with regulatory standards and budget targets.

With a focus on quality improvement and education, you will lead meetings, drive clinical initiatives, and support after-hours care to uphold excellence in patient-centered care across the program.

Qualifications

• Licensed RN in New York State.
• Nurse Practitioner with NYS license and DEA preferred.
• Minimum of three years clinical hospice experience.
• Experience in clinical/quality improvement and regulatory compliance.

Responsibilities

• Coordinate and lead hospice team workflow and ongoing patient care.
• Ensure regulatory compliance with Hospice Conditions of Participation.
• Collaborate on quality improvement and education initiatives.
• Manage department operations, budgeting, staffing and performance evaluations.
• Oversee after-hours hospice coverage and interdisciplinary plan of care.
