Qualifikationen

• Graduate of an accredited nursing program and active RN license.

Overview:The Hospice RN Case Manager is responsible for coordinating and providing high-quality hospice care to patients. This role involves developing individualized care plans, managing patient visits, collaborating with the interdisciplinary team, and ensuring compliance with hospice standards and regulations.
