Qualifications

• Must have previous experience in a hospice home care setting with two years of management or supervisory experience.
• A valid driver’s license.
• Current CPR certification.

Responsibilities

• Receive case referrals and review available patient information related to the case.
• Assign appropriate hospice personnel to referral case as needed.
• Review and evaluate each case through home visits, conferences, and records review.
• Discuss and verify impressions to promote effective performance.
• Conduct clinical record reviews and communicate findings to the Clinical Director.
