Bristol Hospice in Chicago, IL, is seeking a Clinical Supervisor to ensure care and services are delivered appropriately and to supervise clinical personnel. This role requires a Registered Nurse license and prior hospice/home care supervisory experience.

Qualifications

• Registered Nurse license in good standing to practice professional nursing in the state.
• Two years of management or supervisory experience in a hospice/home care setting.
• Excellent observation, verbal, and written communication skills.
• Valid driver’s license and CPR certification.

Responsibilities

• Receive case referrals and review patient information to determine hospice/home care needs.
• Assign appropriate hospice personnel to referral cases as needed.
• Confer with Medical Director and Attending Physician regarding eligibility for services.
• Review and evaluate cases through home visits, conferences, records review, and clinician services.
• Discuss and verify impressions and guide clinicians to improve home care delivery.
