VIA Health Partners in Charlotte, NC, a not-for-profit hospice and palliative care provider, seeks a Hospital Team Manager to supervise the Hospital Team and ensure patient care meets organizational and regulatory standards.

You will lead an interdisciplinary team, oversee scheduling, performance appraisals, recruitment, onboarding, and coaching, and collaborate with the EMR team to maintain documentation and recertification.

Qualifikationen

• Bachelor’s Degree in related field required; Master’s Degree preferred.
• Minimum 2 years’ related management experience in home health/hospice, oncology, or medical-surgical.
• Proficiency in Microsoft Office required.

Aufgaben

• Oversee an interdisciplinary hospice hospital team and ensure quality patient care.
• Supervise, evaluate, and mentor team members; conduct performance appraisals.
• Recruitment, orientation, and onboarding of new staff.
• Develop and revise the plan of care through IDG meetings and planning activities.
• Ensure compliance with policies, procedures, and regulatory requirements.
• Collaborate with EMR team for documentation and recertification processes.
• Manage scheduling, time sheets, and payroll for direct reports.
• Promote continuous quality improvement and effective problem solving.
