Job summary

VIA Health Partners in Rutherfordton, NC seeks an experienced leader to oversee an interdisciplinary hospice team and ensure high-quality patient care. The role emphasizes supervision, hiring, mentoring, and plan-of-care development through IDG meetings while maintaining compliance with policies and regulations.

Ideal candidates have a Bachelor’s degree (Master’s preferred) and at least two years of progressive management experience in hospice or healthcare, with strong Microsoft Office skills

Qualifications

• Bachelor’s Degree required; Master’s preferred. Or the equivalent combination of education, certifications, training, and work experience.
• Minimum 2 years’ related experience with increasing management responsibilities in hospice/healthcare.
• Proficiency in Microsoft Office is required.

Responsibilities

• Oversee and direct an interdisciplinary hospice team.
• Ensure clinical care meets patient and family needs.
• Supervise and evaluate staff with timely performance appraisals.
• Assist with hiring, orientation and mentoring of new employees.
• Develop and revise the plan of care through IDG meetings and planning activities.
• Ensure service compliance with VIA policies and regulatory requirements.
• Collaborate with EMR team to support recertification and documentation.
• Maintain professional relationships with providers, patients, families, and vendors.
• Attend meetings and participate in quality assurance audits as needed.
