Job summary

VIA Health Partners in the United States is seeking a proactive Clinical Team Manager to oversee an interdisciplinary hospice team. You will guide clinical care, ensure services meet patient and family needs, and drive quality through standard policy adherence.

The role includes supervising and evaluating staff, assisting with hiring and onboarding, and coordinating care plans through IDG meetings. You will ensure compliance with VIA policies and regulatory requirements while maintaining

Qualifications

• Bachelor’s degree in a related field required; Master’s degree preferred.
• Equivalent combination of education, technical certifications, training, and work experience accepted.
• Minimum 2 years’ related experience with increasing management responsibilities.
• Proficiency in Microsoft Office required.

Responsibilities

• Oversee and direct an interdisciplinary hospice team.
• Ensure clinical care meets patient and family needs.
• Provide supervision and evaluations; conduct performance appraisals.
• Assist with hiring, orientation, and mentoring of new employees.
• Develop and revise the plan of care through IDG meetings and other planning activities.
• Ensure the service complies with VIA policies, procedures and regulatory requirements.
• Provide palliative care and symptom management consultations.
• Ensure documentation meets standards and quality initiatives.
• Collaborate within EMR systems to support recertification and discharge decisions.
• Monitor reports and data to manage the team effectively.
• Maintain professional relationships with providers, patients, families and vendors.
