Qualifications

• Must have previous experience in a hospice home care setting with two years of management or supervisory experience.
• A valid driver's license and current CPR certification are required.
• Excellent observation, verbal, and written communication skills are necessary.

Responsibilities

• Receive case referrals and review available patient information related to the case.
• Assign appropriate hospice personnel to referral case as needed.
• Confer with Medical Director regarding eligibility for services.
• Review and evaluate each case and provide recommendations.
• Assist in screening and interviewing new employees.
