Job summary

Bristol Hospice - High Desert in Klamath Falls, OR is seeking an experienced Clinical Supervisor to lead clinical staff and ensure top-quality hospice/home care services. This full-time role requires RN licensure, supervisory experience, and strong communication skills.

The position focuses on coordinating referrals, guiding clinicians, and maintaining compliance with hospice standards. The office is based in Klamath Falls with responsibilities across the local hospice network.

Qualifications

• Registered Nurse license in good standing to practice professional nursing in the state.
• Two years of management or supervisory experience in hospice home care.
• Enthusiasm for providing the best possible care to patients and their families.
• Understanding of the hospice philosophy and issues of death and dying.
• Excellent observation, verbal, and written communication skills.
• A valid driver’s license.
• Current CPR certification.

Responsibilities

• Receive case referrals and review patient information related to case, including home visits, to determine hospice/home care needs.
• Assign appropriate hospice personnel to referral case as needed.
• Confer with Medical Director and Attending Physician regarding any questions as to an individual’s eligibility for services.
• Review and evaluate each case through home visits, conferences, records review, and services provided by clinicians.
• Review patient’s medical diagnosis, prognosis, medications, procedures and clinical course.
• Guide clinicians to promote more effective performance and delivery of quality home care services.
• Available during operating hours to assist clinicians as appropriate.
• Conduct clinical record reviews and communicate findings to the Clinical Director and hospice personnel.
• Assist in screening and interviewing process of new employees and make recommendations for employment.
• Assist in orientation of new employees, as needed.
• Assist in planning, implementation and evaluation of in-service and continuing education programs.
• Assist in formulating, revising, implementing and evaluating organization policies, procedures, goals and objectives.
• Comply with accepted professional standards and principles.
• Assess the health needs of the community and assist in developing plans to meet needs.
• Participate in public relations and community activities to promote the organization’s role.
• Promote customer service orientation to all hospice personnel.
• Participate in quality improvement teams per the QAPI Program.
• Perform other duties as delegated by the Director of Patient Care Services.
• Additional duties as assigned.
