Job summary

St. Croix Hospice in Huntley, IL is seeking a Manager of Clinical Services to lead our clinical team with compassion, vision, and accountability.

You will mentor staff, coordinate patient care, and partner with an interdisciplinary group to deliver high-quality, patient-centered hospice services. The ideal candidate is a registered nurse with hospice or home health experience and at least two years of supervisory leadership.

Qualifications

• RN license in the state of practice (Illinois).
• 2+ years hospice or home health clinical experience.
• 2+ years of supervisory or management experience.

Responsibilities

• Oversee coordination of patient care and assign appropriate staff to each case.
• Support clinicians with real-time guidance, mentoring, and clinical problem-solving.
• Conduct case reviews, home visits, and clinical audits to ensure high-quality care.
• Manage hiring, training, supervision, and evaluation of clinical staff.
• Assist in ongoing staff education, orientation, and performance improvement efforts.
