St. Croix Hospice is seeking a Manager of Clinical Services in Huntley, IL to lead a dedicated clinical team delivering patient-centered hospice care. You will shape strategic care plans, mentor leaders, and drive a culture of excellence and empathy to ensure comfort, dignity, and high-quality care when it matters most.

Qualifications

• RN license in the state of practice.
• 2+ years hospice or home health clinical experience.
• 2+ years supervisory/management experience.

Responsibilities

• Oversee coordination of patient care and assign appropriate staff to each case.
• Support clinicians with real-time guidance, mentoring, and clinical problem-solving.
• Conduct case reviews, home visits, and clinical audits to ensure high-quality care.
• Manage hiring, training, supervision, and evaluation of clinical staff.
• Assist in ongoing staff education, orientation, and performance improvement efforts.
