Job summary

Care Initiatives Hospice is seeking a Clinical Manager - RN to provide comprehensive, compassionate care for patients at home, in long-term care communities, assisted living residences, or hospitals. You will lead an interdisciplinary team and ensure the highest standards of comfort and dignity.

The role requires an active RN license, CPR, hospice or palliative care experience, and leadership experience. Home access to the internet is needed to coordinate care and documentation.

Qualifications

• Active RN license (or ability to activate) in the state(s) in which you operate.
• Active CPR certification.
• Hospice or palliative care experience.
• At least one year in a leadership or charge nurse position or equivalent.
• Two years of recent clinical experience in hospice, oncology, home health or public health nursing; hospice experience preferred.
• Home access to the internet.
• CHPN certification preferred, or ability to obtain within 24 months of employment.

Responsibilities

• Lead and coordinate an interdisciplinary clinical team to deliver comprehensive hospice care.
• Meet patients and families where they are to provide compassionate, dignified care and support.
• Ensure high-quality documentation, compliance, and communication with care teams and families.
