Job summary

The Care Team Hospice is seeking an experienced Hospice Clinical Liaison (RN/LPN) to strengthen partnerships with hospitals, skilled nursing facilities, senior living communities, physicians, and other healthcare providers. This is a community-facing role that blends clinical expertise with relationship development to connect patients with the right hospice services.

As a Hospice Clinical Liaison, you will travel within the assigned geography, provide hospice education and clinical consultation,

Qualifications

• Active and unrestricted RN or LPN license required.
• Two or more years in hospice, palliative care, home health, or related healthcare experience preferred.
• Strong knowledge of hospice care and Medicare Hospice Benefit.
• Excellent communication, presentation, relationship-building, and customer service skills.
• Ability to work independently, manage territory, and maintain presence with referral partners.

Responsibilities

• Build and maintain strong relationships with hospitals, physicians, SNFs, and other referral partners.
• Provide hospice education, consultation, and guidance on eligibility, disease progression, and symptom management.
• Maintain a consistent presence in assigned facilities and support timely referral and admission coordination.
• Collaborate with care teams to remove barriers and improve referral-to-admission conversion.
• Conduct clinical screenings and eligibility assessments within RN scope of practice.
